1. Payment is expected when services are rendered. In order to focus on our patients' needs, customer service and minimizing costs, we do not offer billing options.
2. Payment options include cash, check, debit card/credit card, and Care Credit. All cards must be signed by the owner of the card.
3. If your pet is insured, integrative veterinary services are often covered by your insurance provider. Please check with your insurance company directly to determine what may be covered under your plan.
*If you need to cancel or reschedule your appointment, please provide 48 hours notice to avoid a late cancellation/no show fee of $50. If you are more than 15 minutes late for an appointment, you will be asked to reschedule your appointment and will be charged a $50 late cancellation fee. Dr. Theisen typically has a waiting/cancellation list for any available appointments, and in the event you are unable to keep your appointment, we would like to be able to offer that appointment to another animal in need. We appreciate your understanding with this policy.
